PRN POSITION SUMMARY: The Social Worker is responsible for providing direct social services to patients and family members in the patient’s home, to ensure that the medically related emotional and social needs of the patient are met or maintained. This position is PRN.This position is also responsible for the needs assessment, care planning, problem resolution, discharge planning, client advocacy, and completion of documentation per State and Federal legislations. The Social Worker embraces all components of the job, fostering an environment that allows CHHARC to provide optimum care to our patients. ESSENTIAL FUNCTIONS: Implements the social service program to meet the medically related emotional and social needs of the patients and their family. Assists with problem solving, provides crisis counseling, bereavement counseling, information resources and referrals as appropriate. Participates in discharge planning, completes patient assessments and development and implementation of care plans for each patient on case load. Develops working relationships with the patient, their families, and staff for further understanding of significant social and emotional needs underlying the patient’s health problem. Serves as a liaison and an advocate for the patient and their family. Provides information to the patient and their families regarding financial concerns, including general information regarding Medicare, Medicare and application for Medicaid and other financial assistance programs.Provides contacts to assist with the development and/or maintenance of positive self-image and promotes continued growth of each patient’s potential. Completes assessments and documentation as required by state/federal regulatory requirements, as well as department policy.Develops and maintains community contacts, utilizing available community resources to meet the patient’s individual needs. Provides staff orientation and periodic training concerning resident rights, advanced directives, and psychosocial issues or aging. Works in cooperation with co-workers including utilizing consultation Master Social Worker (MSW) to meet the needs of patient as needed. Organizes and prioritizes work to complete responsibilities according to plan and within established time frames. Participates in organizational and developmental goals and performance improvement activities. Maintains a positive outlook toward employer, work responsibilities, and patients. Maintains knowledge of and adheres to established organizational and departmental policies and procedures. Acts as a representative of CHHARC at all times, demonstrating professionalism in dress and appearance, courtesy, poise, attitude, interactions. Completes required training and in-services promptly.

EDUCATION: Bachelor’s Degree in human services related field required. Graduate of an accredited school of social work preferred.
